Prediction of extubation readiness using lung ultrasound in preterm infants

چکیده

We aimed to test the hypothesis that a lung ultrasound severity score (LUS) and assessment of left ventricular eccentricity index interventricular septum (LVEI) by focused heart can predict extubation success in mechanically ventilated infants. conducted prospective study on premature infants less than 34 weeks’ gestation. LUS was performed postnatal Days 3 7 an investigator who masked infants’ ventilator parameters. LVEI pulmonary artery pressure (PAP) were measured at Day 3. A receiver operator curve constructed assess ability success. Spearman correlation between PAP. total 104 studies 66 infants; them 39 had mild 65 moderate-severe disease. predicted with sensitivity specificity 91% 69%, respectively. Area under 0.83 (CI: 0.75–0.91). did not differ succeeded failed extubation. It correlated PAP during systole (r = .66). conclude predicts preterm whereas correlates high

Prediction of Extubation readiness in extremely preterm infants by the automated analysis of cardiorespiratory behavior: study protocol

BACKGROUND Extremely preterm infants (≤ 28 weeks gestation) commonly require endotracheal intubation and mechanical ventilation (MV) to maintain adequate oxygenation and gas exchange. Given that MV is independently associated with important adverse outcomes, efforts should be made to limit its duration. However, current methods for determining extubation readiness are inaccurate and a significa...

Markov-based Modeling of Respiratory Patterns for Prediction of Extubation Readiness in Preterm Newborns

In this work, we applied Markov chain modeling to gain insight into the respiratory patterns of extremely preterm newborns being considered for weaning from breathing support (extubation). We show that semi-Markov chains provide more robust modeling capability than Markov chains, especially when dwell time durations in states are very long and are not distributed exponentially. We also introduc...

High-flow Nasal Cannulae in Very Preterm Infants after Extubation

BACKGROUND The use of high-flow nasal cannulae is an increasingly popular alternative to nasal continuous positive airway pressure (CPAP) for noninvasive respiratory support of very preterm infants (gestational age, <32 weeks) after extubation. However, data on the efficacy or safety of such cannulae in this population are lacking. METHODS In this multicenter, randomized, noninferiority trial...

The Effect of Oral Motor Intervention on Oral Feeding Readiness and Feeding Progression in Preterm Infants

Background: Advances in neonatal intensive care have markedly improved survival in preterm infants. These babies need hospitalization due to the immaturity of the respiratory and digestive systems. The timing for the initiation of proper feeding in premature infants admitted to neonatal intensive care units (NICUs) is an important challenge for physicians, parents, and...

Ultrasound diagnosis of nephrocalcinosis in preterm infants.

The incidence of nephrocalcinosis in very low birthweight (less than 1500 g) premature infants was assessed by ultrasound scan and analysis of urine. Three of 36 infants had nephrocalcinosis. All had been receiving long term frusemide for bronchopulmonary dysplasia with simultaneous fluid restriction. Urinary investigations showed no consistent findings in babies with nephrocalcinosis.
